При посещении центра Аструм несовершеннолетним ребенком в сопровождении взрослых, не являющимися законными представителями требуется доверенность на основании: статья 20. Федерального закон от 21.11.2011 N 323-ФЗ (ред. от 28.12.2022) «Об основах охраны здоровья граждан в Российской Федерации» (с изм. и доп., вступ. в силу с 11.01.2023), требуется доверенность (скачать образец доверенности), от законных представителей ребенка ( родители либо опекуны).
Данную доверенность можно направлять на официальную электронную почту ООО «ЦЛС и И Дантист» (dantist-nf@mail.ru).
Центр оставляет за собой право отказать в приеме несовершеннолетнего без наличия данной доверенности.

«Об утверждении правил предоставления медицинскими организациями платных медицинских услуг» уведомляем Вас о том, что: ООО «Центр лазерной стоматологии и имплантологии Дантист» не является медицинской организацией, участвующей в реализации программы государственных гарантий бесплатного оказания гражданам медицинской помощи и территориальной программы государственных гарантий бесплатного оказания гражданам медицинской помощи,
*кроме того, медицинские услуги по проведению медицинского осмотра согласно требований Приказа Минздравсоцразвития РФ от 12.04.2011 г. № 302н
«Об утверждении правил предоставления медицинскими организациями платных медицинских услуг» уведомляем Вас о том, что: ООО «Центр лазерной стоматологии и имплантологии Дантист» не является медицинской организацией, участвующей в реализации программы государственных гарантий бесплатного оказания гражданам медицинской помощи и территориальной программы государственных гарантий бесплатного оказания гражданам медицинской помощи,
